Evaluations: 

I would like to complete the School Consultation Request form on-line and have it submitted by email (preferred)

I would like to download School Consultation Request form and complete them by hand. 

I wish to access the on-line Parent Request for School Consultation

I wish to download the parent questionnaire so I can facilitate its completion before the appointment.

 

 

School consultation procedure:  

Dr. Turley requests that information be collected before consultation times are scheduled.  Why?  Parents and professionals who have invested time gathering information are much more likely to keep appointments.   There is a high demand for the consultation hours, so no-shows waste a valuable resource. 

  1. Contact the intake coordinator at the school system to check availability.  (see below)
  2. Complete the School Consultation Request Form (on-line, or manually).   (Brockton Public Schools:  Use the CCT request form available from Vincent Cerce at VINCENTCERCE@brocktonpublicschools.com or (508) 580-7521 
  3. Gather additional documentation (testing reports, disciplinary referrals, home studies)
  4. Have parent (s) complete the Parent Questionnaire for initial referral (on-line or manually). 
  5. Schedule a time with the coordinators as noted below:
