Setting up an initial appointment:
Dr. Turley's private practice is currently full, but consider completing an evaluation request anyway. Your referral information is provided entirely at your discretion, but is kept confidential. Records are kept in compliance with new federal rules. If he has a cancellation, or as time begins to open up, he may be able to call on short notice.
School funded evaluations: Dr. Turley protects some time for school-funded evaluations because these do not require follow-up appointments. He will answer specific consultation questions in a thorough typed report, and will bill the school directly, if the school authorizes payment in advance. School adjustment counselors and parents are asked to complete the on-line questionnaires and referral forms, and gather any other information that might be pertinent to the consultation question (behavioral reports, testing reports, information about current school performance, and any scored behavior checklists like the Conner's Checklist, or Basic Assessment of Child Behavior (BASC)).

Information to bring with you:
Prior to the assessment session: complete the on-line triage questionnaire, and gather any other information that might be pertinent to the consultation question.
Follow-up sessions: Think about any questions that you want to ask, and information you wish to convey. Since time is often of the essence, please strive to be well prepared and organized. Complete the on-line medication appointment form.
If school data has been requested, be sure to touch base with the teachers, administrators, or the adjustment counselor. Invite them to e-mail Dr. Turley at doctor@JeffTurley.com. Running Short of Medication?

Dr. Turley gets a lot of these contacts, and has a limited and variable amount of time to deal with them. Give him as much notice as possible. Dr. Turley may require knowledge of the next appointment before filling prescriptions. His policy is to see patients who are doing well (requiring little adjustment of medication) every 3-6 months. Dr. Turley would prefer that you fill out the on-line medication appointment form when requesting refills, so that he can document an update of your status.
Controlled substances: must be ordered with an original prescription. Accordingly, Dr. Turley will need an address to which to send the prescription.
These include all of the traditional stimulants for ADHD (Ritalin, methylphenidate, Concerta, Metadate, Dexedrine, dextroamphetamine, Adderall, mixed salts of amphetamine).
Non-controlled substances: Dr. Turley may or may not have ready access to patient records. Information needed for prompt call-ins of medication refills: Medication Name, Dose in mg (as well as how many times taken per day), patient's full name and date of birth, the current number of days until the medication runs out, and a pharmacy number.
