Dr. Jeff Turley is a board-certified Child and Adolescent, and Adult Psychiatrist in practice since 1985. 

Dr. Turley's mission is to provide the highest quality child, adolescent, and adult psychiatry to the community.  He uses a humanistic, whole-person perspective.  He prides himself on the clarity of his communication and the thoroughness of his reports.

 This tool was designed by Dr. Turley and programmed by Mike Turley as a public service. 

Purpose:  to provide detailed clinical information (medications, symptoms, current status) about a patient to an evaluating or treating mental health professional.   

Who can use this form?  Respondents can be patients (self-report), parents, or other individuals with detailed information about the patient's status.

Who gets the information?  The form processes information and sends a report as an email attachment to any email address the respondent specifies. 

(Uncheck Dr. Turley's name if you want the information sent only to other parties you specify). 

Teachers:  Follow this link to the Teacher Report  This form also provides detailed information about a student's behavior status by email to Dr. Turley and/or any third party that you choose.
